How to Compute Failure index for composites

Hello all
Im trying to compute Composite Failure Index for a particular laminate, with respect to certain Failure theories ( for Ex: Tsai-Hill) 
1) how do i get it done from solver, so i get to plot Failure index for Laminate.
2) I did run into the Derived results section, but was not sure about the expression i created was correct, where can i get the Index reference for terms used in the Derived result.

I have attached a sample file, that needs to be checked for failure index under Tsai hill

    If the solver is OptiStruct, you need to request CSTRESS, CSTRAIN and CFAILURE Outputs, besides specifying the allowables into your material card.

     

    The attached model is an example including failure calculation.

    Also by requesting PARAM,ALLFT,YES, OptiStruct will calculate as many failure criterea as possible (Hill, Tsai-Wu, Hoffman, etc).
