This is the first time I met the problem.

use CheckEL Card to ignore element quality

Element # 421640, element type TETRA. this element need to convert in CTETRA.

unfortunetely, none of these will solver your issue.
What version of OptiStruct are you using?
I would try a few things.
- add the control card PARAM, NLMON, DISP in order to wirte a more detailed output and an additional H3D file showing each iteration >> then load the H3D file and take a look if there's anything strrange going on; Also check the _nl.out file for more detailed description of errors.
- Add the control card PARAM, EXPERTNL, YES and give it a try to check if the convergence improves
Try to change to LGDISP loadcase (activate NLPARM(LGDISP)), and also add NLADAPT (DTMAX field) in order to have a better control in the load increment. If I recall correctly, this is already available for Small Disp Loadcases in newer versions. But depending on what version you're using it will be available only for LGDISP.

But there is no information showing the model could not pass the element quality check. Moreover, I run with CheckEL but it still shows the problem.

yes, please.
the best option to figure it out is to add PARAM,NLMON,DISP and inspect the new H3D and the nl.out file. It will show you which node has some large errors and then you can investigate further.
