First on the mesh - I would assign the surface between the inner and outer volumes to have the same mesh size as the inner volume - then grow from that size into the outer volume. It looks like that 'interface' currently has the larger size of the outer volume.
The Log file shows there is reverse flow on the outlet surface. On that surface, activate backflow conditions - so you can assign properties of the reversed/incoming flow - and assign the treatment for the eddy viscosity of the entering flow. I would suggest trying area average or exiting area average.
