How to model CV joint between two solid flanges as indicated in the above pic?

Depending on your application, MotionView is perfectly capable of creating this type of system. See below for a simple mock-up of your diagram:
Joints Only:
The constraints on this model would be cylindrical joints and CV joints. Springs would allow the deflection and a Motion would provide the rotational velocity desired.
