Hyperworks student license application form

In the reply mail for HyperWorks 14.0 installation request, there is a license application forum with a column ' Name of the CAD/CAM/CAE Faulty'.What does that mean?

    Dear Student,

     

    You need to provide the 'Name & Contact details of CAD / CAM / CAE Faculty '

     

    You have to provide the details of the same in excel sheet.
