Control logic of PSFB error

i try to make a simulation of PSFB base on official example and make a bit modification. but i found some error of below logic control circuits.
V38 turns high, though inputs of SR-flip flop V39 V42 has no changes. it seems abormal and i cannot understand the logic.
anyone can help me?
i use Psim V9.1.

    You are looking at the incorrect voltage probe.
