Refine by Pattern Shrinking Elements instead of Refining?

Hi, 

 

In the pictures shown, I am using the mesh refinement toolbox as instructed by the guide, however instead of a refined mesh, it is also shrinking my elements:

 

 

Result:

 

 

 

Can anyone explain?
