Volume Tetra from the 3D panel creates a 2D mesh in an area of the object

Volume Tetra from the 3D panel creates a 2D mesh in an area of the object.

I created a 3D mesh with volume tetra (second order tetra6 elements). Then I realized that in an area of the object I was analyzing the software creates a 2D mesh. I cannot understand  why. Can anyone help me? Thank yo

    Make sure you have a fully enclosed solid before tetmeshing.
